NDIS Social Work

Chartwell’s NDIS Counselling and Social Work Support Service is designed to assist individuals with disabilities in navigating the challenges they may face in their daily lives. Our team of trained and experienced social workers provide a safe and supportive environment for individuals to explore and address these challenges, developing strategies and plans to achieve their goals and improve their overall well-being.

Our services include support with managing mental health, addressing relationship issues, capacity building, coping with stress, anxiety, depression, and grief, as well as developing plans for mental health support and community engagement. These services can be provided in the comfort of one’s own home, in the community, or our office.

NDIS Counseling

Our NDIS counseling and support services are designed to assist you in coping with everyday stresses and challenges, so you can participate fully in everyday life. Our friendly counselors will work in partnership with you to:

  • Identify your goals
  • Identify strategies that may improve the quality of your life
  • Building understanding of your mental health and conditions

We are dedicated to providing you with the support and guidance you need to improve your overall well-being and live your best life.

We can provide NDIS registered counseling services to you if you are:

  • 12 years and older
  • Have capacity building (improved daily living) services in your approved NDIS plan.
Counseling services can be claimed under Capacity Building (improved daily living). We apply pricing in line with the NDIS price guide.

Advocacy

At Chartwell, we believe that change is possible and courageous. Recovery is possible. And we believe that our clients have the courage and strengths to make it happen. We work with our clients to identify areas in their lives that they would like to change and provide strategies to make those changes possible. Our therapists are prepared to have courageous conversations, challenging clients to move closer to their goals.

Managing life changes or a diagnosis

We’re here to support individuals with disabilities and their families through any significant changes they may be facing. Whether it’s working through a new diagnosis, adjusting to a new environment, or finding new ways to navigate challenges, we’re here to help take those steps together.

Therapeutic Intervention

Building social skills and managing emotions can be a challenge for some individuals. That’s why we offer therapeutic intervention through activity-based approaches to support increased focus and engagement. This can help individuals develop the skills they need to navigate social situations and manage their emotions more effectively.
